The story prologue found in the original American manuals:

Save the Chaos Emeralds!
Could this be the end of South Island? That evil maniac Dr. Robotnik has another twisted idea to take over the world! This time he wants to steal the mystical Chaos Emeralds.
The Emeralds are the source of vitality for all living things. But in the hands of rotten Dr. Robotnik, they could be turned into nuclear bombs and laser weapons!
The evil doctor has already snatched the Red Chaos Emerald, causing the other Emeralds to lose balance and fly into a parallel universe. Now South Island is sinking into the ocean. Sonic and "Tails" will have to use all their super speed and super stunts to stop Dr. Robotnik and restore harmony to the universe!
"OK, Dr. Robotnik," says Sonic, "You're charged with contempt of hedgehog. There's no time to lose. Gotta slide, Clyde!"

Zone descriptions[]

Zone Description
Turquoise Hill Zone Speed through oceanside loops and tricky platforms loaded with Rings. Bounce off the Boing-o-Bots' spring noggins for high-flying Ring grabs. Look out for spike traps and the Lady Bug Boss!
Gigalopolis Zone Smash through walls with the Super Spin Dash. Watch where you jump, or Dr. Robotnik's traps will make you a chump! Metal-o-Turtles and bugs are out to get you. Fight the Bead Worm Boss to clear the zone.
Sleeping Egg Zone Bash upward through solid blocks. Make huge spring-loaded mega-hops. Ramps send you soaring. Weird optical illusions and Dr. Robotnik's Veg-o-Bots try to do you in. The Bouncy Boss Robot is heavily armed. Watch out!
Mecha Green Hill Zone Mad mastermind Dr. Robotnik even has Coconut-Bots, so keep an eye on the palm trees. Look out for traps everywhere. Jump to save yourself from the deadly molten metal. Stay on top of the Tree Crawler Boss.
Aqua Planet Zone Dive into an underwater quest with fierce currents and treacherous tunnels. Hold your breath and grab air bubbles to survive. Countdown numbers appear over your head to warn that you're running out of air. The Sphere-o-Bot Boss awaits.
Electric Egg Zone Breakaway floors, laser guns and indestructible booby traps are lurking dangerously in this final zone. Conveyer belts make for slippery progress. The Laser Walker can get you with its bounce shots.

South Island, the island of peace...
Here lie the super-material of dreams "Chaos Emeralds". They have unlimited power, but that's why they can be used as terrifying weapons depending on their use... Now, aiming for these "Chaos Emeralds", the evil genius scientist Doctor Eggman has started to take action again!
One day, while Sonic and Tails were out on an adventure, they heard an unpleasant rumor.
"Eggman's got one of the Chaos Emeralds?!" When they rushed back to South Island, the island of peace was already in chaos! When one of the gems was taken, the rest of the "Chaos Emeralds" were also scattered somewhere, and the island, which had been supported by the power of the jewels, was about to sink into the sea!!
"This time it's my victory. The world belongs to me, Sonic!!"
"Eggman, I'll definitely take back the Chaos Emerald! "Let's go buddy!" "Roger!"
Sonic and Tails set out to thwart Eggman's ambitions and save the sinking South Island. Will it be Sonic and the others who have the last laugh, or will it be Eggman...?

Zone Description
Turquoise Hill Zone Turquoise hills with endless blue skies and green meadows. Before the full-scale battle, first warm up at the local South Island! Collect more and more Rings and defeat the enemies!
Gigalopolis Zone "Gigalopolis", a future city with ultra modern buildings. With the beautiful starry sky in the background, Sonic and Tails go berserk here too! Let's dash and cross the bridge that leans diagonally.
Sleeping Egg Zone A mysterious island floating in the sky. It's actually a fantasy world created by Eggman! It's really strange that there are statues of Eggman here and there on the island. Be careful not to fall...!
Mecha Green Hill Zone Eggman has transformed Green Hill on South Island into a world of machines!? ...When we rushed over, the flowers, trees, and soil were all machines! I won't forgive you, Eggman!!
Aqua Planet Zone A water planet in an alternate dimension... Strange creatures are roaming around. Don't forget to take a breather after going underwater.
Electric Egg Zone We've finally arrived at Eggman's electric base! However, it's not so easy to get out of here, as the computers control everything. Don't give up until the end and do your best! The showdown with Eggman is coming!!

Robotnik's at it Again!
For a year after the six Chaos Emeralds were restored to South Island, all was peaceful and quiet... except for the occasional sonic boom as Sonic the Hedgehog and his friend Miles "Tails" Prower practiced their speed runs.
Then one day, the island was rocked by a terrible explosion. Great chasms opened up in the ground. Strange new landmarks sprang into being. The animals โ€” including Sonic and Tails โ€” immediately began searching for the cause. It was Flicky the Bluebird who had the answer: "The Chaos Emeralds are missing from the North Cave!"
Tails frowned. "If the six Emeralds are separated, there's no balance, and chaos'll leak out all over the world! And if we don't find them fast, South Island will sink into the ocean!"
At that moment, a strange round flying machine roared overhead. Clutched in large pincers on its underbelly was the red Chaos Emerald! All the animals could see of the pilot was his bushy mustache and big red nose, but his nasty laugh was unmistakable โ€” Dr. Robotnik!
"This time nothing can stop me! Once I have the other five Chaos Emeralds in my possession, I will rule the world! HAA, ha, ha, ha, haaaa!" Then the evil Dr. Robotnik and his flying machine vanished over the horizon.
"Not if we find'em first," said Sonic. "C'mon, Tails. Let's get those Emeralds!"

Turquoise Hill Zone Race through loops, corkscrew twists and hidden passageways. Ride moving platforms over pits filled with spikes. Spring high in the air over traps and terrain, and keep on the lookout for Robotnik's badniks!
Gigalopolis Zone City lights glitter in the night as you spring from level to level. Race through shining corridors, then jump from top to top and grab those Rings!
Sleeping Egg Zone Hop onto a Pogo Spring and bounce through mazes of blocks. Break blocks to reach monitors and open secret passageways. This is no place to be caught napping. Platforms that appear solid might not be. One wrong step, and it's a long way down!
Mecha Green Hill Zone Steel platforms replace the old Green Hills, and mechanical palm trees drop deadly bombs on you if you get too close. Don't let yourself sink too deep into the river ooze, or you won't get out.
Aqua Planet Zone Pop open the hatches to underwater tunnels and the current will carry you along, but beware of spears ahead! If you go without air for too long, you'll see a countdown above your character's head. When the timer reaches zero, you lose a Life. Replenish your air supply by jumping to the surface.
Electric Egg Zone You have to move fast to get safely through the dark corridors of Dr. Robotnik's lair. Grab all the Rings and bonuses you can - you'll need them when you meet with the evil doctor himself, in an arena he has prepared specifically for you...

Grab those Rings and Items, and keep on the lookout for Robotnik's Badniks!
Gigalopolis Zone City lights glitter in the night as you spring from level to level. Race through shining corridors, then jump from top to top and grab those Rings!
Sleeping Egg Zone Grab a Pogo Spring and bounce through mazes of blocks. Break blocks to monitors and secret passageways. Looks can be deceiving, and platforms that appear solid might not be. One wrong step, and it's a long way down!
Mecha Green Hill Zone! The Green Hill Zone has gone mechanical, and the skies are gray with pollution thanks to Dr. Robotnik. Don't let yourself sink into the sludge in the river or you won't get out. The coconuts in the trees are explosive, so if one drops onto you, get out of the way!
Aqua Planet Zone Push open the hatches in the underwater tunnels and the current will carry you along, but beware of the traps ahead! If you go without air for too long, you'll see a countdown above your character's head. When that count-down reaches zero, you lose a Life. Replenish your air supply by jumping to the surface or taking a deep breath from big air bubbles that drift up from certain blocks.
Electric Egg Zone Guided lasers guard the corridors of this Zone, and you have to move fast to outrun them. Dodge the robot bombs that drop from the pipes. Use the D-Button to guide your passage through networks of pipes, and grab all the Rings and Items you can.

Finally you'll meet with Robotnik himself, in an arena he has prepared especially for you...
